PreRegistrationIf you wish to register with the Practice click on the link below to open the form. When you have completed all of the details, print off the form and bring it to the Surgery. You will be asked to sign the form to confirm that the details are correct.

ALL New Patients must complete the registration form and a new Patient Questionnaire. These must be brought into the surgery yourself along with two forms of ID, (One photographic) either a drivers licence or passport, together with a utility bill, bank statement or other official proof of residence (less than three months old).

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

If you are taking repeat medications it is essential when you register that you bring to the Surgery either a print-out or the right hand side of a recent prescription from your previous GP detailing your repeat medications.  The Dispensary will not be able to dispense repeat medications without verification.
